If you are a supervised visitation provider and want to apply for the court’s resource list, you must meet the qualifications of a provider as outlined in Standard 5.20, Uniform Standards of Practice for Providers of Supervised Visitation and Cal. Fam. Code § 3200.5 (external site ). In addition, you must be willing to certify, annually under penalty of perjury, that you have received the training identified under Standards of Judicial Administration 5.20 (external site ).
If you are interested in being included on the Supervised Visitation Resource list and you meet the training and qualifications you must submit all of the following forms:

Resource lists are updated annually. If a complete application is not submitted each calendar year,
your name will be removed form the list of authorized providers.
Reports must be filed with the Family Law clerk’s office. Each time you file a supervised visitation monitor report with the court, you must affix form FL-324(P) Declaration of Supervised Visitation Provider (Professional) (external site pdf ) to the front of the report.
