Miscellaneous Self-Help Packets

Self-Help form packets generally include a group of forms that are required to be completed in order to either begin or complete a case.

Request to Waive Court Fee Waiver If a person can’t afford to pay the court costs and fees. English

Care Act

The CARE Act creates a voluntary agreement that the respondent participate in Behavioral Health Services. The agreement can include court ordered treatment, support, and a housing plan.
